Developing Effective FMEAs for Process Industry Applications

As used in the process industry, FMEA and RCM are focused toward achieving intrinsic reliability of process plants. The quality and success of FMEA depends to a large extent on capturing all potential failure modes. This requires inputs from experienced personnel in plant operations, maintenance and functional specialists. These individuals are special resources and their involvement needs to be utilized effectively for a successful study.

This presentation describes a methodology for achieving the objective effectively (in terms of both the quality of the analysis and the time required to perform the analysis) in the context of functional failures of systems and equipment in process industries. The discussion will include an example of an FMEA study carried out for a lube and seal oil system built for typical turbo machines in a process industry.

Key Words: FMEA, Process, Industry, Functional, Failures, Method, Capture, Failure Modes, Ishikawa
